Monet David, MS, LPC  and Allison
Buller, PhD, LPC takes United Healthcare and UMR health insurance plans.  and private pay.  

Mallory Gothe, MS, PLPC, and Ashley Rush, MS, PLPC are private pay. 

Monique Cohen, APRN, PMHNP- BC takes United Healthcare, UMR, and Blue Cross Blue Shield health insurance plans. 

Dr. Perkins' psychological evaluations are private pay only. 

We all file out-of-network for most insurance companies, but all billing questions can be directed to our Billing Department, by calling the number above.
